Process and Procedures

When it is known that an employee will be (or has been) absent, due to personal illness, for 10 or more consecutive work days:

  • OEARM sends a letter to the employee with a Disability Slip for completion by the treating provider.
  • The employee is responsible for keeping OEARM informed of Return to Work (RTW) status. 
  • Before returning to work, the employee must submit a current, completed Release to RTW form from their treating provider directly to OEARM.
