Process and Procedures
When it is known that an employee will be (or has been) absent, due to Personal Illness, for 10 or more consecutive work days:
- OEARM sends a letter to the employee with a Disability Slip for completion by the treating physician.
- The employee is responsible for keeping OEARM apprised of Return to Work (RTW) status.
- Before returning to work, the employee was submit a completed Release to RTW form from their treating physician.
